Perhaps the cap is supposed to be an AC rated cap, and a DC rated cap was 
installed?

To give you an idea, when an AC cap rated at 100V is required, an equivalent 
DC cap should be at least 600V, and sometimes not even that's enough.

If it comes in again, put in an AC cap.
